You may submit the following tax documents: 1040, W2, K1, or 1099s.

Please note that to qualify based on income, the requirement is $200K/yr for each of the past two years as an individual (whether you filed as single or jointly), or $300K/yr including spousal income (or spousal equivalent) for each of the past two years if you filed jointly.
